二、定标地点、方式及疫情防控要求

（一）定标地点和时间

为保证定标全过程公平、公正、公开，我公司拟于2021年8月20日15时00分（北京时间）在雅安市政务服务和公共资源交易服务中心开标室进行。
|
||||
</p>
|
||||
<p style="text-indent:2em;">
|
||||
（二）定标方式
|
||||
</p>
|
||||
<p style="text-indent:2em;">
|
||||
1.中标候选人参与方式
|
||||
</p>
|
||||
<p style="text-indent:2em;">
|
||||
经查询，本次中标候选人注册地址（以工商登记地址为准）包括雅安市内11家、成都市46家、宜宾市2家，其他省市12家，考虑到最近全国疫情防控的高压态势，为做好疫情防控措施，避免过多雅安区域外人员入雅，我司拟通过电话联系+网上公示的方式通知注册地址在雅安市范围内的11家公司在规定时间之前进入定标现场（由法定代表人到场的，应携带投标人企业营业执照副本（或正本）原件、本人身份证原件；由委托代理人到场的，委托代理人必须是投标人本单位人员，应携带投标人企业营业执照副本或正本原件、委托代理人身份证原件、委托代理人连续6个月在该投标人单位的养老保险缴纳凭证原件或提供由社保部门出具的委托代理人在该投标人单位连续6个月参保的证明原件。中标候选人同时须提供以上资料复印件一套并加盖公章），否则视为放弃中选资格。

对注册地址未在雅安市范围内的60家公司，我公司在定标前开通网络视频会议室，邀请60家公司在视频会议前半小时进入视频会议室，对超过截止时间（2021年8月20日15时00分）未进入视频会议室的，视为放弃中选资格。自愿放弃参加定标的公司，应在定标前一日通过邮寄或现场提交方式向招标代理公司提交《自愿放弃参加定标会议的承诺书》，否则视为放弃中选资格。

||||
参与视频会议的公司，要求在接到电话通知时，提供与会人员的姓名、参与会议的钉钉账号，并在会议前将参与人员授权委托书、公司营业执照复印件、法人及被委托人身份证复印件盖章后寄送至指定地点。我公司在会议前将以此为依据在钉钉中添加好友，并核对各账号实名注册信息，核对一致后方认可各公司参会人员及账号。在视频会议召开前半小时，逐一核对参会人员与纸质资料一致性，核实无误后方认可参会人员参与资格。

2.定标抽取程序
|
||||
</p>
|
||||
<p style="text-indent:2em;">
|
||||
将71枚乒乓球从1到71的顺序进行编号，分别对应在全国公共资源交易平台本项目中标候选人公示中的第一名至第七十一名中标候选人名称（网址：http://www.yaggzy.org.cn/）。例：1号乒乓球对应中标候选人第一名四川鸿瑞一城建筑工程有限公司，10号乒乓球对应对应中标候选人第一十名四川锦诚瓴宇工程有限公司，我公司根据中标候选人实际到场及视频连线情况，将对应的乒乓球放入雅安市政务服务和公共资源交易服务中心摇号设备中。

||||
我公司通过随机抽签方式在到场的中标候选人中确定摇号机操作人员，并由该人员通过摇号设备随机抽取一枚乒乓球，该乒乓球编号对应的中标候选人即作为中标人，并经参与会议现场的所有人员确认无异议后（现场主持人宣布“XX公司为中标人，若对此结果有异议，请现场提出，未提出异议视为各候选人对中标结果无异议”），由现场所有人员签字确认中标结果，视频会议全过程进行录像保存。若存在异议，则按照规定程序处理。

（三）疫情防控要求
|
||||
</p>
|
||||

|
||||
按照《雅安市应对新型肺炎疫情应急指挥部办公室关于进一步加强疫情防控工作的紧急通知》（雅疫指办发【2021】20号）等相关文件规定，为积极配合好疫情防控措施，抽取现场全程佩戴口罩，所有到场中标候选人均需完成新冠疫苗接种，且行程码为绿色。
|
||||

三、监督方式
|
||||

||||
抽取过程在雅安市政务服务和公共资源交易服务中心开标室进行，在集团法务监审部及工程部的监督下进行，由雅安市政务服务和公共资源交易服务中心监控系统进行全过程监控。

五、注意事项

1.应试者做好个人疫情防控相关事宜。面试期间全程佩戴口罩，并主动出示本人“天府健康通”防疫健康码信息（绿码），主动接受体温测量。如有发热、发烧、咳嗽等异常症状者，须携带7日内的有效核酸检测报告，因应试者个人原因不能进入考场，由应试者自行承担后果。

2.因故不能参加面试或面试时因疾病等个人原因不能继续进行的，视同自愿放弃面试。
|
||||

3.应试者要自觉遵守考场秩序，服从考场工作人员安排。面试结束后，不得再进入考场或在考场附近逗留，不得将已答试题的内容以任何方式告诉其他应试者。
